Applicant Pre-Screening Form

  • The grant programs we offer provide financial assistance for individuals who want to improve themselves through training and employment.
  • A prescreening form is a questionnaire or form used to gather preliminary information about individuals before they undergo a specific process, such as applying for a job or receiving a service. The purpose of a prescreening form is to filter out individuals who may not meet certain criteria or qualifications, saving time and resources for both the applicants and the organization administering the process.

Workforce Development Board (WDB) Application

  • Funding opportunities are accessible for various circumstances outlined in a prescreening questionnaire, aiming to identify eligible programs for individuals. Respondents’ answers remain confidential and are handled with the utmost discretion. The questionnaire prompts individuals to select relevant situations from a list, including being in a low-wage job, experiencing disability, homelessness, childcare or transportation issues, struggling with literacy, technology, or English language proficiency, and dealing with substance use challenges involving alcohol, street drugs, or prescription medications.
  • Complete this application if:
    • You want help getting a job but do not wish to enroll in training right now.
    • You need help with training and training-related expenses.
  • Includes career goals, demographic information, employment and experience, household information, and suitability information
